Angina pectoris is a type of temporary chest pain or discomfort that occurs as a result of decreased blood flow to the heart muscle which is usually not a heart attack but a sign of increased risk for heart attack that is generally characterized by symptoms such as squeezing pressure, heaviness, tightness or pain in the chest that can occur sudden or recur over time.
Nictoris 5mg Tablet contains Nicorandil which belongs to the group of medicines called Potassium channel activators. It is used in adults to manage and reduce painful signs of angina pectoris who cannot take heart medicines called beta-blockers or calcium antagonists.
Nictoris 5mg Tablet is not recommended for use in patients with severely low blood pressure, low blood volume, or heart problems (such as cardiogenic shock, left ventricular failure with low filling pressure, or cardiac decompensation).
Before taking Nictoris 5mg Tablet, inform your doctor if you have lung problems such as pulmonary edema (a condition marked by fluid build-up in the lungs), hepatic impairment, or kidney problems.
Nictoris 5mg Tablet is not recommended for use in pregnant and breastfeeding women. It is also not recommended for use in children and adolescents (below 18 years of age). Consult your doctor before taking it.

Angina pectoris (heart-related chest pain)
The pharmacological action of Nictoris 5 mg involves dilating arterioles and large coronary arteries by opening potassium channels, which stimulates guanylate cyclase, leading to venous vasodilatation. This mechanism reduces both preload and afterload, thereby improving coronary blood flow.
Side effects may include transient headache, flushing, dizziness, nausea, vomiting, weakness, hypotension, and reflex tachycardia at high doses.
The recommended dosage for adults is typically within the range of 10 to 20 mg twice daily. The initial starting dose is usually 10 mg twice daily, with adjustments made based on individual patient needs, response, and tolerance, up to a maximum of 40 mg twice daily if necessary. In patients prone to headaches, an even lower starting dose of 5 mg twice daily may be considered. Special considerations apply to the elderly and those with hepatic impairment, where lower starting doses and cautious administration are advised.
Regarding interactions, while no significant pharmacological or pharmacokinetic interactions have been observed in studies with Nictoris 5 mg, caution is advised, especially when co-administered with other vasodilators, tricyclic antidepressants, or antihypertensive drugs, as these may potentiate its effects.
Certain contraindications exist, such as hypersensitivity to Nictoris 5 mg, cardiogenic shock, acute myocardial infarction with acute left ventricular failure and low filling pressures, hypotension, and concurrent use with phosphodiesterase-5 inhibitors due to the risk of severe blood pressure drop.
Caution is warranted in pregnant and breastfeeding women, as there is limited data on its safety in these populations.
Additionally, precautions should be taken in patients with hypovolemia, low systolic blood pressure, acute pulmonary edema, or during pregnancy. Overdose effects may manifest as peripheral vasodilation, leading to decreased blood pressure and reflex tachycardia.
Nictoris 5 mg should be stored in a cool and dry place, protected from light.
